Shipping Instructions to Manufacturer (AVI Survival Products) for Service Work
For shipping purposes, self-inflating life rafts are classified as DG (Dangerous Goods)/Hazmat (Hazardous Material) due to compressed gas cylinders (CO2) and pyrotechnic devices (flares).
DG/Hazmat shipments to manufacturer must include:
Order Form, MSDS (Material Safety Data Sheet) & Shipping Labels

Option 1:

Freightquote has relationships with several Freight Companies. Freightquote.com will research the most efficient and Cost Saving shipping method, execute the required paperwork, and pick up the shipment at your location.
Option 2:
Call a freight company of your choice. Provide them with the required paperwork. It is recommended to insure the raft/equipment at full value.
Other Freight Companies:
AAA Cooper, ABF Freight, Bax Global, Benton Global, Best Overnight Express, R&L Carriers, Roadrunner Express, Watkins Trucking, Yellow Freight, etc. For example: Go to the ABF Freight website and click on "Bill Of Lading Creation Form" (located left of center on their homepage). Complete the BOL (Bill of Lading) E-Form while following their online instructions and print.
Steps:
a) Fill out the BOL and print. (Refer to the MSDS for DG/HazMat information)
b) Print the MSDS. (Must be included with your shipment)
c) Print the DG/HazMat Shipping Labels and attach to TOP of shipping carton.
d) Complete the Manufacturer Service Information Form and place in shipping carton with raft.
e) Notify Freight Company shipment is ready for pickup.
f) Make sure to render the BOL and MSDS to the carrier along with your package(s).
Where is the information I need to complete the BOL?
DG Information to include on the BOL can be found on the MSDS.
Other Information to include on BOL:
-- Description: "Deflated life raft"
-- Weight
-- DIMS (Dimensions of box)
-- Insurance value
-- Type of Hazardous Material: "Life-saving appliances, self-inflating, (UN2990)".

Option 3:
Certified DG Shippers can Call 1-800-GO-FEDEX (800-463-3339). Ask for Dangerous Goods Hotline, OR go to the FedEx website: www.fedex.com (Complete and print the Shipper's Declaration of Dangerous Goods Form). You can, also, call or visit your local Fedex Branch (Print the DG Shipping Labels and attach them to TOP of box). Use Manufacturer's MSDS as a reference.
Note: Fedex does not ship DG to/from all destinations. Certain countries will not accept DG shipments. FedEx will not ship DG unless you are a Dangerous Goods Known Shipper.
